Yes. There were some big problems in the lexer embedding joined  
sections code recently.  There are a number of bugs filed in different  
areas with slightly different symptoms, so I'm not sure if they're all  
related to the same root cause, but I suspect they are since the Ruby  
or JavaScript embedding code hasn't changed lately.

I think it's likely that the bugs you list below, as well as others  
like 138649 are all duplicates. Hopefully this is fixed now; I believe  
a fix was also backported to the 6.5M1 branch (details are in the  
above report).
