ModernCV style - address block and name header missing

I am trying to create a CV using the ModernCV style on Mac OS Lion, Lyx 2.04 and MacTex 2012.


Although I can add and entry and it will compile OK, there is no address block or name header. 

There are some instructions on the Wiki ( http://wiki.lyx.org/Examples/ModernCVClassIssues) about centering the name and title and leaving more room for the address, by editing the .sty file which may be relevant, but the instructions don't match the current moderncvcasual.sty file, and I am not sure what I need to do, if indeed it is relevant. 

In desperation, I actually created the CV that I needed in latex, and copied the personal info into the Lyx preamble, so I am assuming that the required information is available to Lyx.

Is there anything obvious I should be looking at.

BTW, don't be misled by me moving to Latex to create my CV, I don't actually know much about Latex, its just that when things don't work in Lyx its sometimes quicker to find an example in Latex, and edit it directly. than try and work out what is happening with Lyx - but this is a temporary solution as I primarily want to work in Lyx.
