[GIT PULL ofed-1.5] cxgb3 upstream bug fixes

Please pull from:

ssh://vlad@.../~swise/scm/ofed_kernel ofed_1_5

This pulls in all the latest upstream bug fixes for cxgb3 and adjusts 
the backport patches accordingly.
